To help your first visit go smoothly, please download, fill out, and print out the paperwork. At the time of your evaluation, bring the paperwork with you and arrive at least 15 minutes early. You may need to answer additional questions regarding your medical condition or history.

This process requires additional data input (in paper form or via tablet) at your first visit and during re-evaluations to measure the effectiveness of our interventions.
