  • What should I wear for my physical therapy appointment?

    Please wear comfortable clothing like active wear and tennis shoes, as your session may involve exercise. For lower extremity injuries, please wear or bring shorts.

  • What should I bring for my physical therapy appointment?

    Please bring your photo ID, insurance card and doctor’s referral if your insurance requires a referral. Any x-rays, MRIs or reports that will help us better understand your situation are also welcomed. Don’t forget to print off your new patient paperwork and fill out prior to your first visit.

  • Do you accept my insurance plan?

    We work with a majority of health plans and insurance companies to give you the broadest, most convenient access to our services. There are some insurance plans, however, that we are not able to work with directly and still maintain what we believe to be an acceptable standard of care for you. When you call to make your appointment, we will be happy to contact your insurance company and verify your physical therapy benefits.

  • How far in advance should I schedule?

    Since our therapists’ schedules fill quickly, it is wise to make your appointments as soon as you know when you will be available. We recommend that you schedule at least 1-2 weeks ahead of the date you would like to be seen.

  • How often should I schedule my physical therapy appointments?

    This depends on several factors: the nature and severity of your problem, your physician’s recommendation, your therapist’s assessment, your schedule, and your insurance coverage. Your physical therapist will come up with the best plan to meet your needs and discuss this with you during your first visit in therapy.

  • What happens if I miss my appointment?

    We understand things come up, which is why we do our best to accommodate your busy schedule. We need at least 24 hours advanced notice for cancellations.

  • Can I switch to another ProActive location for convenience?

    Yes, you can be seen at the location that best fits your needs. All of our expert physical therapists will be able to provide you with the exceptional healthcare experience you deserve, and the ongoing care you desire.
